چکیده

Anaesthesia in children with a neuromuscular disease is challenging the anaesthesiologist. In this group occur frequently perioperative complications from muscle weakness, respiratory depression and cardiac involvement. Lack of experience is the reason for uncertainty. Knowledge of the pathophysiology and relevant pharmacology of the disease, preoperative development of an adequate treatment plan with consultation of other disciplines, and close monitoring of the patient are extremely important. The various diseases can be divided according to the location of the disorder: pre-junctional, junctional, post-junctional. Many of them are described in this article. Only few patients are susceptible for malignant hyperthermia. Rhabdomyolyis can be induced by inhalational anaesthesia and succinylcholine in many of these diseases, whereas in most a prolonged effect of muscle relaxants exist and succinylcholine may induce myotonic reactions. The complications and anaesthetic effects in many neuromuscular diseases are described. Anestezjologia i Ratownictwo 2013; 7: 39-52.

Perioperative management of the paediatric patient with coexisting neuromuscular disease.

Children with neuromuscular diseases present a wide range of clinical manifestations and clinical implications for the anaesthesiologist. Neuromuscular diseases in children affect muscle strength by either directly weakening the muscle fibrils or indirectly by a degenerative nerve supply and weak neuromuscular junction. Of the more than 200 neuromuscular disorders known, the vast majority are g...

Rocuronium for muscle relaxation in two children with Friedreich's ataxia.

Friedreich's ataxia is a rare hereditary neurodegenerative disease caused by a defect in the gene that encodes a mitochondrial protein called frataxin. We report the use of rocuronium 0.6 mg kg(-1) in two adolescent girls with Friedreich's ataxia undergoing propofol-sufentanil-oxygen-air anaesthesia for spinal surgery. Neuromuscular transmission was monitored using acceleromyography, and onset ...

Synergism between atracurium and vecuronium in infants and children during nitrous oxide-oxygen-alfentanil anaesthesia.

This study was undertaken to see if infants are more sensitive than children to a combination of atracurium and vecuronium in an equipotent dose ratio: (microgram: microgram) 5:1 in infants and 4:1 in children. We studied 15 infants (1-11 months old) and 15 children (3-10 yr old) during nitrous oxide-oxygen-alfentanil anaesthesia. Neuromuscular function was recorded by adductor pollicis EMG. An...

Ketamine monoanaesthesia for diagnostic muscle biopsy in neuromuscular disorders in infancy and childhood: floppy infant syndrome.

The anaesthetic management of children with neuromuscular diseases giving rise to hypotonia is associated with a variety of problems. Ketamine alone was given by the intravenous or intramuscular route to 32 children with Floppy Infant Syndrome for diagnostic muscle biopsy. The patients aged between three months and 12 yr and weighing 3.2 to 28 kg were studied over three years (1986-1988). The s...

Neuromuscular disease and anaesthesia.

Diseases affecting the neuromuscular system are numerous and not uncommon (for classification see Appendix). Almost all of the drugs used in anaesthesia have either a direct or an indirect effect on muscle, and so an understanding by anaesthetists of the neuromuscular diseases is essential in view of the universal requirement of muscle relaxation during surgery. The intention of this article is...
